The Children’s Hemiplegia and Stroke Association (CHASA) has teamed with researchers at Northwestern Medicine Feinberg School of Medicine, Department of Physical Therapy and Human Movement Sciences, to recruit participants for a research study.

• The purpose of this study is to understand motivators and barriers for participating in cerebral palsy related research and considerations influencing potential participants.
• You will be asked to complete an online survey consisting of up to 60 questions.
• We expect that you will be in this research study for approximately 20-30 minutes of time.
• The primary potential risk of participation is minimal.
• The main benefit of being in this study is informing researchers of how to make participation in future research studies more convenient and enjoyable for you.
Why am I being asked to take part in this research study?
We are asking you to take part in this research study because you or your child has a diagnosis of cerebral palsy.
How many people will be in this study?
We expect about 1500 people will be in this research study.
What should I know about participating in a research study?
• Whether or not you take part is up to you.
• You can choose not to take part.
• You can agree to take part and later change your mind.
• Your decision will not be held against you.
• You can ask all the questions you want before you decide.
• You do not have to answer any question you do not want to answer.
