We Can Help With Your IEP (Individualized Education Plan)

Examples of Individualized Education Programs for students with a diagnosis of hemiplegia, hemiplegic cerebral palsy, or hemiparesis and related disabilities.

IEP and psychological report for Preschool student, age 2, with hemiplegia

IEP for Pre-Kindergarten student, almost 3 years old, with hemiplegia, reduced attention span, gross motor delays, delays in self-help and safety skills

IEP for Preschool student, 3 years old with right hemiplegia (gross and fine motor delays), delayed social skills and difficulty with attention, recall and focus

IEP for Pre-K, age 4, with hemiplegia diagnosis

IEP with goals appropriate for either K or early 1st grade, student age 5 with hemiplegia

IEP for Early Childhood student, age 5, with hemiplegia, language delays, gross and fine motor delays, and slight cognitive delays

IEP for Kindergarten student, age 5, with right hemiplegia (fine and gross motor difference right side); attention/focus issues; executive function, impulse, organization and planning issues; vision field cut; vision motor difference (similar to dyslexia); difficulties with sequencing, understanding content/reading comprehension, recall; and feeding delays

IEP for 4th grade student with hemiplegia, specific learning disability, orthopedic impairment, and other health impairment
